I n a previous issue of Heart, Scholten and colleagues presented data to show that a biphasic external atrial defibrillation shock waveform offers no significant advantage over a monophasic shock waveform in current clinical practice. However, there is evidence in this study and elsewhere to suggest that a biphasic defibrillation waveform is ‘‘better’’ than monophasic. The sense that this should be so may be engendered by the past experience of electrophysiologists with ventricular defibrillators and the decrease in defibrillation threshold that was seen with the advent of biphasic shock waveforms in these devices. The question is whether the perceived advantages of biphasic waveforms for atrial defibrillation can be translated into clinical benefit for patients?

The Role of Biphasic Shocks for Transthoracic Cardioversion of Atrial Fibrillation

The modern generation of transthoracic defibrillators now employ impedance compensated biphasic waveforms. These new devices are superior to those with monophasic waveforms and practice is currently switching to biphasic defibrillators for the treatment of both ventricular and atrial fibrillation. However, there is no universal guideline for the use of biphasic defibrillators in direct current ...

Comparing the efficacy and safety of a novel monophasic waveform delivered by the passive implantable atrial defibrillator with biphasic waveforms in cardioversion of atrial fibrillation.

BACKGROUND The passive implantable atrial defibrillator (PIAD) (with no battery or discharging capacitor and powered transcutaneously by radio-frequency energy) delivering a novel monophasic low-tilt waveform is more efficacious than the standard monophasic waveform at atrial defibrillation. Standard biphasic (STB) waveforms, however, are more efficacious and safer than monophasic waveforms. Th...

Biphasic versus monophasic shock waveform for conversion of atrial fibrillation.

Cardioversion of atrial fibrillation (AF) using traditional monophasic shock waveform is unsuccessful in up to 20% of cases, and often requires several shocks of up to 360 J. Based on the success with biphasic shock waveform in converting ventricular fibrillation, it was postulated that biphasic shocks would allow cardioversion with lower energy. In a international multicenter, double-blind, ra...
